Team members will be provided initial facts about an unexpected failure, then evidence will be provided based on their Failure Scene Investigation (FSI) data collection efforts as it is uncovered. Participants will develop hypotheses about how the failure modes could occur and use collected evidence to continually drill down the cause-and-effect chain until they uncover the physical, human and latent root causes
